This Dental Membership Plan is an annual membership plan that allows individuals and families who don't have dental insurance to receive quality dental care at a reduced cost.

With our unique Membership Plan, you will no longer fear the unknown cost of your dental care. Our affordable plan includes the following:

• Comprehensive & six month Recall Exam
• One emergency exam per year (if needed)
• Annual Digital X-rays (4 Bitewings & 2 Periapical, 1 set per year)
• Two Preventative Teeth Cleanings (Deep Cleaning Not Included)
• Individualized Patient Care

Membership clubs are not insurance but a payment arrangement provided by Dr. Leone-Renne DDS PC for services rendered. Memberships are provided exclusively to uninsured patients of our practice and shall not be considered pre-payment for future services or payment for access to discounted services. Rather, our patient members are electing to make regular monthly subscription payments for preventive care instead of paying at the time of each service. Payment for any cosmetic or restorative treatment is due at the time of service. Membership dues must be current in order to receive services.  Membership club discounts may not be combined with any other offers or discounts. It is solely the patient’s responsibility to schedule and keep their appointments. No refunds will be provided under any circumstances, including failure to schedule and maintain appointments. A patient agreement is with the legal entity of Dr. Leone-Renne DDS PC. Fees for dental services may change at any time.
